Caroline Kavanagh

Caroline Kavanagh was a 37-year-old mother from Kilmallock, Co Limerick, who tragically died in 2015 during her second pregnancy due to a misdiagnosed heart attack at Tipperary University Hospital. Despite presenting symptoms of a heart attack, she was discharged without appropriate care and was later found unresponsive at home, leading to the loss of both her and her unborn child, Thomas James.
